Appropriate Hematocrit Levels for Patients to Receive Iron Supplementation

Question:
Is iron supplementation (PO or IV) required when the hematocrit has achieved the target (36%-38%) in dialysis patients receiving erythropoietin?

By NAAC Past President and Nephrologist Allen R. Nissenson, MD.
If the TSAT is <20 or the serum ferritin is <100, I would give iron even if the hematocrit is in the target range. There are independent adverse effects of iron deficiency separate from the effect on anemia.
